Title: Who's guarding death?

I haven't been able to find the death guardian in the death realm, is this a bug? Could someone give me an un-spoilng, subtle hint about where this death guardian guy is?
Why haven't I asked about this in game?

Should be quite obvious once you found the way out of the Death Realm to your resurrection... ;D
