Technical Help: IN GAME bugs (after loading world) / Very annoying bug

I am having a problem with a bug, everytime i leave the town and start travelling down that road I get a good ways up it and then out of nowhere i become link dead, then I log back on, only to fall THROUGH the ground and die from falling, which brings me back to town, and this happens everytime i try leaving the town.
