Technical Help: Problems BEFORE entering the game / log in and player creation problems

first of all whenever I try to create a player it says I am already logged into this server

second of all whenever I try to log in it says \"there is an error in the database on your account. please refer to the forums on how to fix this\" so tell me dammit!
