I know that you go to the death realm or wtvr its called when u die, but do you lose all your items or something when you die?

Well after you die you simply go to Death Realm.

Later on you will be able to sacrifice a magical object to avoid that

I think if you use the /spawn command to get out of the Death Realm then you lose the some of the items in your possession.
