You place it snugly over a nut and the turn.

Here's a tip - if the nut is too hard to turn, place a pipe over the wrench to increase your leverage but be cafeful not to use too much force.

I believe you mean the winch? you must take one of several quests to get access to it and then you right click on the door and use the key icon.

The "Winch" is a building in an enlosed town quarter, which is able to pull goods and travelers up from lower levels of Yliakum... which are not yet implemented, but imaginable. It is restricted due to the high value of many goods.

There are several ways to obtain access to this quarter. One is mentioned on the gate with a note (right-click it, look at it). But there are other less lawful ways too...
